For any forms that are collecting information, please print them out and fill in. Following the instructions on the form to learn how and to whom you should submit the completed form. If you have a computer with the capabilities, you can fill them out and return them as an attachment to an email.


General Forms for Members

  • New Member Information - used for newly baptized individuals or for those placing membership
  • Event Guide and Building Use - instructions for those renting the facilities for weddings, etc.
  • AWANA Store Needs - the children earn "AWANA Bucks" to spend in in the "store" as rewards for memorization. We ask members to contribute items to the store.
